What Is Tooth Resorption?

Feline tooth resorption (TR) — previously called feline odontoclastic resorptive lesions (FORLs) or neck lesions — is a condition in which the tooth structure is progressively destroyed from the inside out or outside in by cells called odontoclasts. It's one of the most common and painful dental conditions in cats, affecting an estimated 30-70% of cats over 5 years of age. The cause is not fully understood despite decades of research.

How It Manifests

Tooth resorption typically starts at the gumline where it's difficult to see without probing. As the lesion progresses, the tooth's hard structure (dentin and enamel) is replaced by bone-like tissue, eventually causing the crown to fracture and exposing sensitive pulp tissue. Cats typically hide dental pain extremely well, so owners may not notice signs until disease is advanced. Warning signs include excessive drooling, dropping food, chattering jaw when eating, and avoiding hard foods.

Diagnosis

Tooth resorption cannot be reliably diagnosed on visual oral exam alone. Dental radiographs (X-rays) are essential — they reveal the extent of resorption within the tooth root, the type of resorption (Type 1, 2, or 3), and guide treatment decisions. This is why dental radiographs during professional cleanings are standard of care, not optional extras.

Treatment Options

Unfortunately, there is no treatment to stop or reverse tooth resorption. The only effective treatment is extraction — removing the affected tooth and any retained root fragments. For Type 2 resorption (where the root is fusing to the jawbone), crown amputation (removing the crown while leaving the resorbing root in place) may be appropriate. Most cats show dramatic improvement in behavior and activity level after painful affected teeth are removed.

Prevention and Monitoring

No preventive measure has been shown to reliably prevent tooth resorption. However, regular professional dental exams with radiographs allow early detection before the tooth becomes acutely painful. Annual or twice-yearly dental radiographs in cats over 5 years old are the best way to monitor for this condition and intervene before advanced stages cause significant suffering.
